Raw egg product were stored above cooked meat product - corrected on site. A pink slime substance was found in three of the four ice makers in the dry storage area. Ice scoop was stored in ice machine with handle in contact with ice. Employee failed to use sanitizer at work station. Deeply scored cutting boards were not resurfaced or discarded as required. Repair ware wash - final ware-wash rinse temperature was at 170degF. Repair disposal. Ware wash needs to be repaired. Food-prep area floors - a non-food contact surface - is not cleaned at a frequency to preclude accumulation of dirt and soil. Food employees in prep area were not wearing a proper hair restraints, such as a hair net or hat - repeat violation. Time in lieu of temperature is being used in the food facility to control pizza - a time and temperature control-for-safety food - without written procedures to verify the disposition of the food. Can opener blade is worn and is subject to creating metal fragments. Deeply scored cutting boards were not resurfaced or discarded as required.

CREEKSIDE CAFE

Regular inspection.

Food facility does not have a certified food employee - the facility has 90 days to enroll an employee in a food safety course that is recognized by the state. There were torn rubber door gaskets on the cooling unit. There was a black and tan residue inside the beer taps. A dog, and water and food bowl for dog, were inside the food facility.

Food employees in the food-prep area were not wearing beard covers. There was label residue on containers for food. The internal temperature of sliced tomatoes, cheese and crab stuffed mushrooms measured 47-49degF - voluntarily discarded. Ambient temperature of line bain marie measured 49degF rather than 41degF or below. Deeply scored cutting boards were not resurfaced or discarded as required. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Thermometers for ensuring proper temperatures of food are not available. Food facility does not have available quat sanitizer test strips or test kit to determine appropriate sanitizer concentration. Deeply scored cutting boards were not resurfaced or discarded as required. Slicer is in close proximity to a sink used for disposal of liquids. Three-bay sink is leaking from sanitizing compartment.

Regular inspection.

No sanitizing chemical was available in the facility to sanitize food contact surfaces. The hand-wash sink does not have water at a temperature of at least 100degF. No sign or poster posted at the hand-wash sink to remind food employees to wash their hands.

Can opener blade is worn and needs to be replaced.

Most cutting boards in the facility are deeply scored and not resurfaced or discarded as required. The hot water used for sanitizing in the mechanical ware-washing machine did not reach 180degF (or 165degF for stationary rack - service-company was called at the time of inspection. There were copious amount of food debris under food equipment at floor/wall junction.

Regular inspection.

Food employee personal use medicine is stored on shelf above food-prep table. The food facility does not maintain food employee certification records as required. Wet wiping cloths in food-prep area were not being stored in sanitizer solution. Sauce, which was cooled, was only reheated to 118degF for hot holding and not 165degF for 15 seconds as required. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in bain marie units. Facility does not have a container of sanitizer set-up to sanitize food contact surfaces. Hand-wash sink faucet is not maintained in good repair - cold water flow is low. Light not shielded or shatter proof in the walk-in cooler. Rear door has a gap at the bottom and does not prevent the entry of rodents or insects.

CHINA INN RESTAURANT

3985 D Columbia Ave., West Hempfield Twp.

Regular inspection.

Employee's personal bottle of water was stored in container of vegetables used for customer food. Meat in the freezer in the back was stored open with no covering. Raw eggs were stored above celery in the walk-in cooler. Egg rolls were being stored in cardboard boxes, which is not a smooth and easily cleanable surface. Duct tape was being used to hold the inside lid of the chest freezer together. Deeply scored cutting boards were not resurfaced or discarded as required.

DUTCH-WAY FARM MARKET

365 Route 41, Sadsbury Twp.

Regular inspection. Out of compliance.

Person in charge is not performing the duties as required by the PA Food Code to maintain active managerial control of food safety. Meat department: Cleaning-type chemicals stored on the same shelf as food and utensils. Seafood department: Shell-stock tags are not maintained for 90 days from the date the container is emptied. Packaged items ingredient labels do not have the sub-ingredients listed on the label. Meat department: Pieces of bare wood are being used in the walk-in cooler to support the condensing unit. Deli department: The quaternary ammonia concentration in the sanitizing solution of the three-bay ware-wash sink was 0 ppm, rather than 200 ppm as stated on the manufacturer's use directions. Meat department: The quaternary ammonia concentration in the sanitizing solution of the three-bay ware-wash sink was 100 ppm, rather than 200 ppm as stated on the manufacturer's use directions. Restaurant: French fry cutter - a food contact surface - had food residue and was not clean to sight and touch. Bakery department: Bulk food storage containers and racks have food residue accumulation. Bakery department: Clean utensils were stored in a manner that does not prevent touching of food contact surfaces. No sign or poster was posted at the hand-wash sinks in the bakery and meat departments to remind food employees to wash their hands. Bakery department: Floor is very sticky and areas under mixers and three-compartment sink has grease and residue accumulation. Ceiling tiles are missing in the meat department dock area and need to be replaced. Roof is leaking water in meat department dock area and needs to be repaired.

Regular inspection. Out of compliance.

Scoop was being stored in the ice bin with handle buried beneath the ice. Potentially hazardous food was stored directly on the floor in the walk-in refrigerator, rather than six inches off of the floor as required. The outside surface of the soda fountain, a non-food contact surface, is not cleaned at a frequency to preclude accumulation of dirt. There was old food or drink residue in the hand-wash sink, indicating uses other than hand washing.

SUBWAY

245 Bloomfield Drive, Manheim Twp.

Regular inspection.

Meatballs and soup were held at 128degF and 119degF respectively in the hot-holding table rather than 135degF or above as required. Lids for small sauce cups were stored with food contact surface exposed.

605 Richmond Drive, Manheim Twp.

Regular inspection.

Food facility has an employee who held a certified food manager certificate; however, the certificate has expired and is no longer valid - employee is scheduled to take the class. Food employees in the food-prep area were not wearing beard covers. There was tan residue on the ice maker deflector plate. There was static dust on the floor-model fan in the baking area. Single-service, single-use articles (sample cups) were stored under plumbing at a hand-wash sink - a prohibited area.

MANHEIM SUNOCO

216 N. Main St., Manheim

Follow-up inspection. Out of compliance.

Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The food facility does not maintain food employee certification records as required - facility has 90 days to complete a state recognized food safety course. One pint of milk being was offered for sale with expiration date of 5/28/15 - new violation. Again there were filth and debris inside the three-compartment sink. Dumpster area still has excessive clutter and debris and old equipment. Dumpster lids were still not closed at the time of this inspection. Exit door located in the back area of the food facility still has a gap and does not protect against the entry of insects, rodents and other animals. Again there was extreme clutter in the back storage area. Ceiling tiles are still missing in the storage area and need to be replaced. There was standing, stagnant water from water leak behind the wall according to the person in charge. Mops still are not being hung to air dry.

PARMA PIZZA & GRILL

There was dried-food residue on slicer. There was food residue in prep-area hand-wash sink. Faucet for hand-wash sink in ware-wash area is not in good repair. Rear screen door of facility is not tight fitting to prevent the entry of rodents or insects.

French-fry cutter has dried food residue accumulation and is not clean to sight and touch. Shelf for cutting board on bain marie unit in the fryer area has old food accumulation present. Storage bins for utensils have dried food and grease accumulation.

Regular inspection.

Food employees in the food-prep area were not wearing proper hair restraints, such as nets, hats or beard covers. There was an excessive amount of old grease build-up inside fryer in the food-prep area. Faucet fixture for hand washing at the sushi-prep area is loose and leaks.

Working containers of glass cleaners and sanitizers were stored above and on the same shelf with food in the storage cabinet under the soda machine and in the walk-in cooler above returns. Facility does not have a placard with ingredients for doughnuts offered for sale. Single-service, single-use articles (cups) are stored in the walk-in freezer directly on the floor, and not six inches above the floor.

There were torn rubber door gaskets on the cooling unit used for milk. Torn, exposed insulation was hanging from the HVAC unit in the storage area. Emergency door located in the storage area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. There was extreme clutter, debris and store items piled in the back storage area. Mops are not being hung to air dry.

Person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Working containers of cleaners were stored above or on and above shelves with food near the doughnut-making station. The food facility does not have the original certificate for the Certified Food Employee posted in public view. Food employees, again in the food-prep area, were not wearing proper hair restraints, such as nets, hats or beard covers. Cup without a handle being is used instead of a scoop in bulk food ingredients. There was old label residue on several plastic food containers. Wire racks for doughnuts stored clean had old glaze on them and were not clean to sight and touch. Single-service, single-use articles (plastic-ware and napkins) were stored in front service area directly on the floor, and not six inches above the floor. There was a food container lid in the designated hand-wash sink indicating uses other than hand washing. The hand-wash sink in the back was blocked by six racks sitting on top of the sink partitions, and not accessible at all times for employee use. Toilet room is not provided with a covered waste receptacle for sanitary napkins. Outside dumpster lids left open. Tile coving is missing at the mop sink, the three-compartment sink and outside the restroom.

Food employee in prep area was wearing a watch. Plastic food storage containers have sticker residue and are not clean to sight and touch. Plumbing system not maintained in good repair - water was leaking from drain pipe at three-compartment sink. Rear door has a gap at the bottom and does not prevent the entry of rodents or insects.

Regular inspection.

In bakery, an employee wiped off hands on her apron. Internal temperature of fried chicken in cold case measured in the 50degF and not 41degF or less - discarded. Ambient air temperature measuring device for ensuring proper cold storage in cold chicken display case is not accurate to plus/minus 3degF. The ambient temperature of the cold chicken display case indicated 57degF while unit thermometer indicated 46degF. Warehouse dock doors four and seven have a gap and do not protect against the entry of insects and rodents. There was food debris in bakery at the floor/wall junctions.
